In accordance with the Federal Fair Credit Reporting Act (Public Law 91-508, 15, U.S.C. Section 1681 ET SEQ., 604-615), all federal provisions and regulations, a consumer report may be requested and obtained for the exclusive use of Bay Area Pools and Spas, Inc. and will not be used for any other purpose than employment. Bay Area Pools and Spas, Inc. may choose to secure and use information contained in a consumer report obtained by a consumer reporting agency when: (1) considering an application for employment, (2) making a decision to offer employment, (3) deciding whether to continue employment (if hired) or (4) making other employment-related decisions. A consumer report may include, information as to, among other things, character, work habits, performance experience, past employment (including reasons for termination), professional license and credentials, or credit/civil/criminal/driving record history. This authorization and release shall remain in effect throughout the duration of my employment with Bay Area Pools and Spas, Inc. and shall be valid whether original, copy or facsimile. I agree and release to hold harmless Bay Area Pools and Spas, Inc. its officers, agents, employees from and against any and all liability resulting from any decision regarding employment or prospective employment with it, based in good faith upon any consumer report. I certify that all information provided below, on my resume and application is correct. I understand that any false statement provided will be considered just cause for rescinding any offer of employment or, if hired, for corrective action up to and including termination, at any time.
